Important Things to Know for 2011-2012

  1. FSA NOTICE: Effective tax year beginning January 1, 2011, over-the0counter medicines or drugs (e.g. Advil, Ibuprofen, and cough syrup) are NOT eligible for reimbursement under and FSA, HRA, or SHA without a doctor’s prescription Insulin is the only medicine that doesn’t require a prescription. Supplies needed for medical care (e.g. contact lens solution, bandages, thermometers) will continue to be eligible for reimbursement. Upcoming enrollment in your employer’s flexible spending account should take into consideration as of 1/1/2011; over-the-counter items will no longer be reimbursable under the Health Care FSA.


  2. IMPORTANT NOTICE: New mandated federal regulations require all dependent socials for any elections made for medical, dental, and/or vision.


  3. Please UPDATE all personal information online to include any change of address, new phone numbers and/or new email information.


  4. MGM is the Third Party Administrator (TPA) for plan year September 1, 2011 through August 31, 2012. If you participate in the FSA and you have the MGM Flex Debit Card, please DO NOT discard. The MGM Flex Card is good for 3 years and costs $10 to replace should you lose it or throw it out. Flex elections do not rollover. PLEASE KEEP ALL RECEIPTS should you be audited by the IRS or asked to substantiate a claim.


  5. NEW DENTAL – Cigna is the new dental provider. All employees will receive a new dental card. This is a passive PPO plan that gives the participant the freedom to choose any dentist. There is a $50 annual deductible and a $1,000 lifetime maximum benefit. Please refer to the plan brochure under Benefits and Forms for more information.


  6. NEW VISION – Block Vision is the new vision provider. Members have a co-pay for the in-network benefits, exam co-pay$10.00 and material co-pay @10.00. Out-of-network vision services are reimbursed up to a certain dollar amount for covered expenses. Exam,. Lenses and frames (sithe plan allowances) are covered in-network once every 12 months.


  7. CANCER – Aflac will remain the cancer plan provider for Crandall ISD. Cancer insurance is designed to be a supplement and pays for many of the costs not covered by your medical insurance. This plan pays in addition to other coverage you may have. The AFLAC plan includes benefits for Radiation/Chemo, Daily Hospital Room Benefit, Surgical Benefit, Bone Marrow and Stem-cell Transplants in addition to many other areas. Rates for this plan are age-banded. Anyone who has the older EMC plan may keep it on payroll deduction.


  8. NEW DISABILITY – Assurant is the new disability provider. Plan outlines and rates are available in the disability section of the benefit guide, which is located under Benefits and Forms on this website. This insurance is designed to provide a monthly income to an individual that is disabled due to an accident or illness. There are different plans available with benefits becoming available from the 1st day of disability to as late as the 180th day. All new amounts or increases are subject to pre-existing condition limitations. Please refer to the plan brochure under Benefits and Forms for more information.


  9. PRE-PAID LEGAL AND IDENTITY THEFT PROTECTION – There are four plan options available with this coverage. Employees may purchase plans including the Legal Shield and Identity Theft Shield.


  10. NEW VOLUNTARY GROUP LIFE – Minnesota Life is the new provider. This plan offers you and your dependents an opportunity to purchase affordable term life insurance on a payroll deduction basis. New this year only, all employees can apply on a guaranteed issue basis (no health questions asked) for 7 times annual salary up to $100,000 for employee, 50% of employee benefit up to %50,000 for spouse and 50% of employee benefit up to $10,000 for children; however, evidence of insurability is required for entrants electing coverage.

    To log in, please click on the EMPLOYEE LOGIN link in the upper right hand corner.

    Username: First 6 letters of your last name, your first initial followed by the last 4 digits of your Social Security number.
    Password: Full last name followed by the last 4 digits of your Social Security number (no spaces).
    Example: Kathleen Roberson Username: robersk1234 Password: roberson1234
