Important Things to Know for 2012-2013

Your benefits plan year runs from September 1st to August 31st of every year.

Financial Benefit Services Toll Free # is (800) 583-6908. Please do not hesitate to call this number should you have any questions regarding your benefits and the enrollment process.

Please UPDATE all personal information online to include any change of address, new phone numbers and/or new email information.

    NEW Carrier! National Benefit Services (NBS) is the NEW Third Party Administrator for Flexible Spending Accounts (FSA) for the plan year beginning 9/1/2012 (replacing MGM).

    • All reimbursement claims should be sent to NBS beginning 9/1/2012, including the 2011-2012 plan year.

    • The Healthcare reimbursement maximum is reducing to $2,400/plan year due to new federal regulations. There are no changes to the Dependent Care.

    • If you currently participate in Health Care or Dependent Care FSA’s, you MUST re-elect a new contribution amount every year to continue to participate. These elections will NOT rollover.

    • All participants of the healthcare FSA plan will receive a new NBS flex card for the plan year beginning 9/1/2012. Please remember to keep your receipts if you use your Flex Card.

    IMPORTANT NOTICE: Mandated federal regulations require all dependent socials for any elections made for medical, dental, and/or vision.

    • TRS (BCBS of Texas) will remain the medical insurance provider for the upcoming 09/01/2012 plan year. For a detailed outline of the benefits offered under your BCBS medical plan please consult the “Benefits and Forms” section of this website.

    • Minnesota Life will remain the Voluntary Term Life/AD&D carrier for Crandall ISD employees. Guarantee Issue is only available for new employees of the District, as long as coverage is elected within the 31 day new hire enrollment period. Crandall ISD provides eligible employees with $10,000 of Base Life/AD&D coverage as an employer paid benefit.

    • Benefits and rates will remain the same for the following coverage types: Cigna PPO Dental, Block Vision, AFLAC Cancer, Assurant Disability, and Minnesota Group Term Life and AD&D.

    Username: First 6 letters of your last name, your first initial followed by the last 4 digits of your Social Security number.
    Password: Full last name followed by the last 4 digits of your Social Security number (no spaces).
